These forms must be filled out and submitted to the Troop as noted;

Form A & B must be submitted to the Troop once per year to be able to participate in any activity.
Form C must be submitted to the Troop for any activity considered "high adventure" or any event lasting over 72 hours in length.
Form D must be submitted for any BSA High Adventure camp such as Philmont.
PADI must be filled out for any SCUBA activity.
